The defective lens must be returned to Visionary Optics to receive credit.
Please include a copy of the Invoice with the returned lens.
The Defective Lens Return Form can be completed & submitted online.
Defective lens claims should be returned with invoice within 30 days of invoice date for 100% credit if reordered with no parameter adjustments. A Defective Lens Return Form must be completed with an explanation of defect or no credit will be issued. A copy of the Defective Lens Return Form and the invoice must be enclosed with all defective lens claims. (Click here to fill out the online form).
